2025 LHCSA & ALP LHCSA Statistical Reports Now Available
The 2025 Licensed Home Care Services Agency (LHCSA) Statistical Report and 2025 Assisted Living Program (ALP) LHCSA Statistical Report are now available through the Healthcare Financial Data Gateway on the Health Commerce System (HCS).
All LHCSAs are required to submit the applicable report(s) by December 31, 2026.
Agencies that opened during 2025 must complete and submit the Agency Form and Registration Form.

Which Report Does Your Agency Need to File?
Community LHCSAs: Complete the 2025 LHCSA Statistical Report.
ALP LHCSAs serving only Assisted Living Program patients: Complete the 2025 ALP LHCSA Statistical Report. You do not need to enter information in the regular LHCSA Statistical Report.
ALP LHCSAs that also serve patients in the community: Complete both reports: the LHCSA Statistical Report for community patients and the ALP LHCSA Statistical Report for Assisted Living Program patients.
LHCSAs operating exclusively as Fiscal Intermediaries (FIs): Complete the 2025 LHCSA Statistical Report.
Deadline
Both reports are due December 31, 2026.
